Description of good practice

Neighbourhood consultation and liaison work prior to the opening of a new service

Mr. X is 38 years old. He has a learning disability and he lived with his family. He wanted to live in an own apartment. To realise his desire he needed a social network. Mr. X has a social worker who supports him at his workplace ant this social worker gave the address of our organisation to Mr. X. We have found a little apartment for Mr. X which was near by his family. In a conversation with all the parties involved we have arranged, that Mr. X can have lunch every day with his family and that his mother will make his clothes – both lunch and clothes for remuneration. In other respects he gets support from our organisation. He learns how to keep house and how to handle with money. Primarily we gave support two times a week and in the meantime we look to him all 2 weeks one time. Beside his family Mr. X has social contacts in his community. In summer he looks to the garden of his neighbour. By his parents Mr. X has a rabbit and he looks to the rabbit every day. Mr. X is well integrated in social life and can live a normal life – this is the aim of our work.
